7 Fit-Out Mistakes That Delay Healthcare Facility Licensing in the UAE

A clinic can be beautifully built and still fail to open on time. The health authorities in the UAE license a facility only after its design, construction, and readiness match what was approved. This article walks through the most common fit-out mistakes that cause delays, based on the published requirements of DHA, DOH, MoHAP, and the Sharjah Health Authority (SHA).

1. Starting construction before the design and other approvals are in place

In Dubai, DHA’s approval process is integrated with the general building approval process, and its guidelines instruct applicants to obtain approvals from other relevant authorities, such as Dubai Municipality, before construction starts. DHA also states that responsibility for compliance rests solely with the applicant, and non-compliances found during or after approval may have to be corrected at the applicant’s expense

In Abu Dhabi, DOH defines its preliminary approval as the approval issued to a healthcare facility to commence establishment. MoHAP requires the architectural plan to be studied and the site plan approved before the initial approval for inspection is issued.

2. Changing the approved layout without informing the authority

MoHAP is explicit: if the approved drawing changes, the Licensing Department must be informed. In Sharjah, the permanent license requires all facility preparations to be completed in accordance with the approved layout. In Abu Dhabi, DOH offers a dedicated service, Request to Change Internal Plans of Healthcare Facility, so layout changes go through the system rather than being made on site.

3. Using a designer who is not prequalified

DHA runs a prequalification process for health facility design consultants authorised to design and submit plans, and its licensing manual requires the floor plan to be designed and approved by prequalified consultants, with room and bed counts as per the DHA Health Facility Guidelines.

DOH applies this to higher-complexity facilities: hospitals, one day surgery centers, fertilization centers, dialysis centers, diagnostic centers, centers with diagnostic imaging and laboratory services, and oncology centers must use a prequalified consultant to submit engineering drawings. Other facility types submit to DOH directly, where drawings are reviewed and stamped by its planning section.

4. Designing to the wrong authority’s standards

Each authority has its own framework. DHA licenses facilities in Dubai with the exception of those located in Dubai Healthcare City. DOH requires providers to adhere to its own health facility design guidelines. A design prepared for one emirate should not be assumed to satisfy another, so confirm the regulator and the current guideline version before design starts.

5. Requesting inspection before the facility is ready

Inspections are staged, and each stage expects readiness:

  • DHA: applicants must confirm the facility is ready for final inspection; if non-compliances are found, re-inspection is required once they are rectified.
  • DOH: the permanent license process includes a 90% inspection and a 100% inspection; specialties or services not ready for the 100% inspection must be suspended.
  • MoHAP: the final inspection requires an active initial approval and compliance with the engineering guidelines and inspection checklist.
  • SHA: the permanent license requires full compliance with design standards, infrastructure requirements, and safety regulations, and successful completion of the inspection. shj

6. Leaving supporting approvals and documents to the end

Several requirements depend on the physical fit-out and take time to obtain:

  • SHA asks for a building completion certificate, approval of the emergency operation plan, waste management registration (WEQAYA), a FANR license where applicable, and an equipment list.
  • MoHAP asks for a medical waste agreement, a valid Civil Defence certificate, a radiation protection license where applicable, and a list of medical equipment.
  • DOH requires approvals from its Monitor and Control Center and Malaffi, plus medical malpractice insurance and a pest control contract.
  • DHA notes that initial FANR registration is required if the facility installs radiology equipment.

If your design includes imaging or a radiation room, plan these from the start.

7. Letting the preliminary approval run out

Initial approvals are time-limited. MoHAP’s initial approval is valid for six months, and it does not imply approval to practice or operate. DOH’s preliminary approval also has a six-month period, and DOH may cancel it if project completion does not exceed 10% after six months. Extensions require a progress report and the reasons for delay. SHA allows extension of the preliminary approval up to six months.

A related mistake is operating too early. DOH states that during the preliminary approval period the facility may not receive or treat patients or promote or advertise health services, and MoHAP states that the facility must obtain the final license before commencing service delivery or employing staff.

Bonus: Check the premises before you commit to them

The first application stage already asks about the location. SHA’s initial approval requires a layout plan, trade name reservation, and an affection plan from Town Planning. DHA’s e-license guide asks for location details matching Ejari and the trade license, and a Dubai Municipality certificate. MoHAP requires an electric elevator if the facility is on high floors. A unit that cannot meet these conditions turns into a redesign problem later, so verify suitability before signing.

FAQs

Can I start the fit-out while the preliminary approval is being processed?
It depends on the authority and the stage. DOH treats preliminary approval as the approval to commence establishment, and DHA expects other authority approvals before construction starts. Confirm the exact sequence for your emirate first.

Do I need a prequalified consultant?
In Dubai, DHA requires the layout to be designed and approved by a prequalified consultant. In Abu Dhabi, it is mandatory for specific facility types, as listed above.

What happens if I fail the inspection?
DHA requires re-inspection once the identified non-compliances are rectified. Other authorities also require compliance before the final license is issued.

Is a trade license enough to start operating?
No. The trade license and the health facility license are separate, and the facility license is required before providing services.

What if my design includes an X-ray or imaging room?
Plan for FANR registration or licensing early, since MoHAP, DHA, and SHA all reference it.
